I am a 65-year-old women with what my doctor calls end stage hip degeneration. I’m otherwise very healthy and try to avoid massive surgery. He would like to perform a total joint replacement. I am wondering if this is absolutely necessary or is there another option?

If you have exhausted all conservative measures, such as physical therapy and medication, the gold standard treatment for end stage hip joint degeneration is a total hip replacement. There is such a thing as arthroscopic surgery, which involves small incisions and robotic arms used to clean up the joint space, however what evidence is out there shows that these are often quickly followed by a total joint replacement surgery anyway with end stage degeneration.
