Many people feel that back surgery should be a last resort. Is it different for cauda equina syndrome?

Yes, studies do show that emergent surgery for cauda equina syndrome is appropriate and has better outcomes than delayed surgery. Compromise of the nerve roots can be permanent if prolonged.
