Is there a procedure for inserting an artificial patella?
A method of attaching a new patella to the patellar tendon?
All the patella replacements I have read about,
have a teflon button attach to an already existing patella -
still attached to the patellar tendon
I had a femoral groove replacement thirty years ago, and need the tibial plateau replaced. I have been avoiding the surgery, waiting for an artificial patella, but it is becoming more difficult to put-off the surgery. My fear has been that six-months after replacing the tibial plateau, a procedure for inserting an artificial patella will become available. I would like to have this taken care of during the same surgery. Without the patellar cam/fulcrum action, the knee collapses after minimal flexure, and the tibial plateau takes a beating descending stairs.

Can you give more information about how your patellectomy affected you? I had one about 15 years ago and have regretted it since the muscles in my leg (quads) gave out about 2000 (7 years AFTER the patellectomy). I would like to wring the neck of all doctors who do this operation as it is the answer to nothing. Doesn't stop pain, creates extreme disability, and generally should be outlawed! Please elaborate on your situation. I am now facing pain in the joint and am scared to have anything else done by anyone. As far as I am concerned, we have not come very far In this country in the treatment of orthopedic medicine. AS for reinserting a patella, I know that if your muscles are shot you may as well forget it. I have looked into this and have been told by many doctors it would be a waste of time. I have suffered life long knee problems and both legs are really not great. I am of a normal weight and was never overweight, and have done literally everything and more for my knees. I feel cursed with absolutely awful knees and legs.
The extensor mechanism has torn four or five times, I finally said screw it and now it is four inches higher than it should be. The circumferential difference between the quadriceps of the two legs approximately eight inches.
Without the patella, squats are out of the question. After a slight flexure, the knee collapses. As a result, to descend steps after the knee bends 20 -30 degrees the knee collapses and I drop to the next step (the other leg had better be ready catch my weight on the next step or I tumble)
The patellar tendon is still present, quasi-functional but virtually useless without the patella. I am looking for a procedure for reattaching either an artificial patella or even a cadaver patella. I do not know of anyone can reattach a patella to the patellar tendon.
Thanks for posting further information about your predicament. I have discovered from two sources, that a cadaver patella would be of no value for my situation (Like you, I looked into that as well), since my quadriceps seem to be extremely atrophied. Also checked into an artificial patella but was told the same thing, i.e., the quads are too far gone. Was also told that even with joint replacement, my quads are so far gone that all the procedure would do is alleviate the pain in my knee joint; I would never walk any better than I now do, and I am afraid it might be worse! I am seeing yet another orthopedic doctor's opinion (not very fond of them since they seem to write you off after they virtually ruin your legs with regard to patellectomy); he is supposed to be one of the best. He even tried to make me believe that I was some sort of oddity (with regard to the muscle atrophy), and of course, I have read many message boards that speak of muscle atrophy in the extreme after patellectomy. Everyone is right about the difficulties in finding a surgeon that is capable of doing a patella implant. I have found one in San Francisco
who has done `9 patella allographs to knees that already had TKR's. He was willing to do mine-lost my patella to trauma- but after reading his paper in an ortho journal, I had second thoughts. All the surgeries failed, and I felt it wasn't a great idea unless it was a last resort. If you are interested, his name is Michael Ries at UCSF.
